PacifiCare Health Systems Inc. was fined $70,000 by the Colorado Division of Insurance for violating state insurance laws and rules in 1995, the Denver Post reported.
The violations include failing to respond to policyholders in a timely manner, using consumer eligibility forms that unnecessarily restricted benefits and taking too long to process claims.
Santa Ana-based PacifiCare is the second-largest managed-health-care provider in Colorado, covering more than 400,000 individuals.
