Eighteen Orange County facilities were cited for poor care in 1997. Survey reports, complaints and citations for 1996 and 1997 show three or more incidents in various categories:

QUESTIONABLE DEATHS
Questionable deaths were noted by state regulators at eight Orange County nursing homes in 1996 and 1997:
Facilities cited and fined for actions causing a death or “substantial probability of death”:
* Newport Rehabilitation Center
* Anaheim Terrace Care Center
* Harbor Health Care
* Windsor Gardens Convalescent Center
Facilities cited for actions affecting “health, safety and security,” and residents died:
* Sun Mar Nursing Center
* Lake Forest Nursing Center
* Port Bay Care Center
Facilities where regulators raised questions about deaths in state survey reports but took no further action:
* Port Bay Care Center
* Fairview Convalescent Center
* Lake Forest Nursing Center
